1) I represent that my animal is in good health, is current on all required vaccinations
(DHLPP, Rabies and Bordetella), is current on Flea/Tick Preventative Medication and has not been ill with any known contagious viruses in the last 30 days.
2) I understand that while my animal is fully vaccinated that vaccines are not 100% fool proof and there is still a risk (albeit minimal) that my animal may contract a contagious virus/disease.
3) I represent that my animal is sociable and has not harmed or shown threatening behaviors towards any person or any other animal. I understand that should my animal display any unwanted behaviors while at the Wilshire Grand Center that Management has a right to request the animal be removed.
4) I agree to disclose any previous or current medical issues/concerns or events that may have an impact in the suitability of bringing my animal to the Wilshire Grand Center.
5) I release Hanjin International Corporation, Morgan Stanley Senior Funding, Inc., as Collateral Agent, its successors and/or assigns as their interest may appear, and Cushman & Wakefield U.S., Inc., from any liability should my animal injure another animal, person and accept medical and legal responsibility of my pet’s actions.
6) I have read and understood the Service and Therapy Animal Rules & Regulations for Wilshire Grand Center and have obtained a copy of the State of California business, consumer services, and housing agency Department of Fair Employment & Housing service animal laws- comparison chart.
7) Emotional Support Animals are required to provide management with and ESA letter prescribed by a licensed therapist.
8) Proof of a pet license is required. A pet license is an additional form of ID that indicates your pet has been registered with your city and vaccinated against rabies. All dogs age 4 months and older are required by law to be licensed in California. Pet licenses are issued as ID tags that must be worn on your pet’s collar at all times.
9) With my signature below, I certify that I have read and understand the agreement and waivers. I agree to abide by the regulations and accept all terms and conditions as set out.
